(Literally, <span class="hanzi">从来</span> means something like "<b>from the beginning</b>," although no one is really thinking that phrase when they use it.) When used in the positive, though, it is customary to put the adverb <span class="hanzi">都</span> after <span class="hanzi">从来</span>.</p>
